High antibiotic usage in adults is corroborated by health-care utilisation surveys done in LMICs that show a strong trend for adults to access community pharmacies as first point of contact and to self-medicate with over-the-counter antibiotics; 39 , 52 , 53 , 54 however, the prevalence of antibiotic usage in children might over-estimate that in adults due to the higher frequencies of paediatric respiratory infections and health-care visits. 37 Limited data on the sales and use of suboptimal (substandard or falsified) antibiotics exists and, therefore, has not been incorporated into our analysis. 55 , 56 The IQVIA MIDAS database is a proprietary database, restricting access and analysis.
As a result, a retrospective analysis showed that the majority of the compounds occupied a narrow range of CNS MPO scores ( Figures and a, 75% between 1.5 and 2.5), and utilizing CNS MPO scores to triage compounds for screening resulted in numerous false negatives as the calculated scores did not correlate with the data observed in the in vitro MDCK and in vivo K p,uu assays ( Figure c–e). On the other hand, and contrary to the initial results observed in the benzazepine series, a strong trend was observed with the tolyl series ( Figure b) between the in vitro MDCK assay and in vivo K p,uu , with 75% of compounds with in vitro ER < 3 demonstrating K p,uu > 0.3 in vivo ( Figure d,e).
Compared to previously reported colorimetric assay-based approaches 29 – 32 , utilizing growth as a simple readout afforded higher throughput which enabled us to observe a strong trend of convergent evolution: multiple NMN + -reducing variants employ very different sets of mutations to achieve the same predicted mechanism (recapitulating AMP) to enhance the NMN + -dependent activity.
