6 A meta-analysis of 18 studies involving 146,372 octogenarians reported a strong trend toward reduced mortality in OPCAB patients (odds ratio, 0.75; 95% CI, 0.56-1.00; P = .05), although statistical significance was not reached. 4 The ability of OPCAB to dampen systemic inflammatory responses, to minimize organ dysfunction, and to reduce physiologic stress may account for its lower 30-day mortality rate. 7 Although OPCAB patients received fewer grafts overall, their ICOR was significantly higher compared with ONCAB patients.
